Epidural infiltration of glucocorticoids and local anesthetics in the treatment of non-specific back pain

Abstract

Background.

Non-specific low back pain (NLBP) is the most common complaint following minor injuries and acute respiratory diseases and the leading cause of limiting physical activity in patients younger than 45 years. This disease is to be a socially significant medical problem, both as a result of continuous incapacity of this group of patients and high cost of treatment.


Research object.

The aim of the research is to evaluate the effectiveness of epidural injection of local anesthetics (LA) and glucocorticosteroids in nonspecific back pain associated with degenerative-dystrophic lesions of various parts of the spine and determine its effect on improving the quality of life of the patients.

Materials and Methods.

The results of treatment of 71 patients aged 36 to 74 years at the Pain Management Center and the Neurological Department of the Municipal healthcare institution “Regional Clinical Hospital – Center of Emergency Medical Care and Disaster Medicine” since 2016 to 2017 for chronic NLBP were analyzed. Patients were divided into 2 groups: the main group – 38 people, who received epidural steroids and LA solutionы as a monotherapy and 33 patients with standard conservative treatment.

Results.

As a result of the therapy, these patients felt low back pain relief, improving of general well-being and life quality. However, the pain level in main group was 1.7 times lower compared to the control group in 6 weeks after treatment. The vital activity and general functioning improved twice as compared with the results in the main group before treatment, but patients of the control group did not achieve the result even in 6 months after the treatment. Summary. Based on the study results we concluded that the epidural injections of the steroid solution and local anesthetic in patients with chronic non-specific low back pain can be used in the modeling of pain syndrome.
